You are being asked to be a part of an anonymous research study to help us in improving the care given to students here at the Student Health Center. Your input is very important to us as we seek to provide the very best care for you and all of our students. If you choose to participate in the study, none of your responses will be linked to you. No one will know whether or not you choose to participate. Participating in this brief online survey should take no more than about 5 to 10 minutes of your time. We would very much appreciate your help!

You must be 18 years or older to participate in this study. If you are a prisoner you may not participate in this study.
